Hello, I was looking at some of my family members and I's older suits and I noticed some don't have a buttonhole on the lapel for boutonnieres, lapel pins, etc. Is this something a tailor can do or is it too much of a hassle? I can just wear another suit with one if I really want to wear an accessory on my lapel.

Of course, it's very simple. Every lapel starts without a buttonhole and tailor or sewer has to do it.

yes a tailor can do it but you really want to be sure that they can do a good job. Ask to see an example. This is the case where a good machine-made buttonhole will be better than a bad handmade buttonhole. It's not something that every alterations tailor can do well.

Yes, unfortunately you won't find many tailors able to do a flawless work these days. I had to refuse a jacket recently because of very poorly made buttonholes.
I'm even considering to choose machine-made next time.
