I think essentially his issue is he is a heel-to-ball 10, but a heel-to-toe 8.5. Seems a bit extreme but everyone is different. In that case, taking 8.5 would result in the ball of his foot sitting too far forward in the shoe, whereas 10 is right for his heel-to-ball length but is obviously going to be roomy in the toe area. It's a larger difference than I've heard about, but could make sense.
