Being in places with sunshine year round....sunglasses are important to me. So far besides a couple others Cartier and Cutler & Gross remain the best for me

are the well made?

what size to go with? i am thinking 54 as my head is large.

I had quite a bunch of 649s. I suggest you measure your favorite glasses (glass width and overall frame length) and compare it to a 649.
Quality is all good, compareable to $200 glasses.
Colorwise I would suggest tortoise, it is dark in lowlight but works with any other color. Black is just black, when the sun is shining, when it's raining... But that is my opinion.

Wearing a pair of vintage Raybans with so-called "comfort" cable temples this summer, but these cable temples are killing me, after a couple of hours wear it's like someone poking a pin through the back of each ear. Any suggestions to make them more comfortable, apart from swapping out the temples? The weight of the vintage construction adds to the discomfort of course - I'm willing to suffer for fashion, but not this much! ;)
