First of all, eat a f'ing sandwich... or four. Enjoy eating whatever you want while you can.

There are lots of stores that sell slim fit shirts that young kids will like... try Bananna Republic, I hear they have that stuff.

mytailor.com and tailorstore.com are also an idea if you want more conservative fabrics.

A tailor should be able to alter anything you bring to him. He might roll his eyes a little depending on the kind of shirt, but he's still getting paid in the end.

I'm 6'6" and frequently buy tall shirts at JCrew and then have my tailor take them in. The key is finding shirts that have long enough sleeves and high arm holes. I've also had good luck with custom shirts from CEGO in NYC.

I feel your pain, I'm about 6'4". Certainly not skinny, I usually take a L or sometimes XL shirt in traditional sizing (or 16.5 in neck size), but that seems to usually mean a 35ish sleeve, which is at least a good inch too short to look even close to fitting. I can pretty much forget about wearing any US brands, unless they offer a 'long' size, or individual sleeve lengths.
Scandinavian brands tend to be cut for taller people. Check out brands like Eton and Steenstr̦ms (both Swedish, although they aren't made there anymore, I believe Рyou can find more info elsewhere on this forum if you do a search). But honestly, they are notoriously over-priced. At what they cost off the rack here in Norway (around 1200 Norwegian, close to $200, and I imagine even more in the States), you can probably get pretty decent made-to-measure instead and still have some change left over. But they do come up on eBay and such now and then, and bargains can be had. I've picked up Eton for as little as $75 or so, and at that price they are good value for money in my opinion. Eton's slim fit line is pretty damn slim (they also have a 'contemporary fit' that's a bit roomier, more like most regular slim fit, ala most of the Jermyn Street brands and such), and I'm told Steenstr̦ms is even slimmer, although I've never tried it personally.
As for Eton, they don't have different sleeve lengths, but at 16.5 the sleeve is about 37, which is pretty much as long as RTW gets, it seems.
Also, Brooks Brothers have sleeve lengths up to 37 on all their extra slim fit dress shirts, so you might want to check those out.

Man this thread made me laugh! Sorry to chime in so late (to the tune of, oh, a year+) but just wanted to let you know I started a new business that aims to offer slim-fitting stylish clothing at an AFFORDABLE cost. We are still young in our development but already have our line of fitted tees available. Coming soon are slim-fit undershirts and "sculpting" undershirts for those wishing to tuck in that gut a tad.

And I lived in AZ for 7 years and hardly heard a homophobic slur. Yeah, it is a red state but I wasn't bashed too hard with my liberal views.

Brooks Brothers Extra Slim Fit. One of the few shirts I buy these days.. Long tails, and slim enough to not need alteration..

I recommend buying shirts from Zara as their shirts are designed for European figures- to get the really slim ones, you usually have to get quite a small neck size - which is ok if you're not planning to wear them with a tie.

Saw a new store called www.XLTClothing.com. They're a Slim & Tall store. I did notice that they don't have jeans yet. But the shirts, Polo, dress shirts, and socks are really nice. I've ordered a good amount from them. They're decent and customer service/response (email) was great.
