From my experience and from reading other people's accounts, any order totaling above $200 USD for END Clothing will get hit with duties. Latest data point - I placed an order for $197 with END in June 2014 shipping to Colorado and was not hit with any extra charges.

I think the total is the LIST price and not the sale price as well (for END). Also both END websites ship from outside the US so are equally subject to duties.

It can take a while. I've had about 8 total orders with End, every single one over $200 (5-6 orders) I got duties 3-6 weeks later.

Duties from End are always ~ 10% for me. I have no problem with it because their shipping is dirt cheap, many other shops charge more to ridiculously more for shipping, some shops don't even deduct VAT and End have easy returns.

You can call the shipping carrier to see if you will be sent any fees. I usually do this and just pay over the phone rather than waiting around for something in the mail.
