     medium will probably be too tight in the chest, I'm 6' 165 39-40"chest and the medium bd was a smidge small up top. MTM is probably your best bet then, but you might try the m bd just in case.

     agree I actually go 1/2 size up, usually 10.5US and wear 44 in CP lows. (and if anyone got the light grey mids in 44 from the corner and it doesn't work out lmk)

     I wear either BD L and AD M, neck is a little big on the BD L so AD M is perfect for me. Sleeve length is slightly different, but otherwise they fit pretty similar. So you could go with either, difference mainly in the neck and sleeves.

     +1 ask in that ps thread. Or search it, there is a great post by zippy in there somewhere comparing the different models. Size varies drastically depending on the particular model/last so we can't give you a generic paul smith size conversion.

     heel definitely fits loose on these, tried a no slip pad around the back and that was some help but I ended up just taking those off and dealing with the heel slip which doesn't really bother me that much. You can try the half insert as mentioned above, but if you wear real shoes for awhile...

     f+b 745 would be an acceptable alternative, I'd say they have a slimmer profile and most definitely a less wide heel (dayton's heel really feels eee width to me). Also, they can be had on sale for $2-300. I actually think I like my 745s better than my daytons even though the daytons are...

     strange, could of just been an oversized last on that model. From my experience I need .5 size larger in NDC than F+B, and they are fairly tts (10.5 most shoes-->43.5/44 NDC). I feel F+B is quarter to half size large (particularly the 745).
